When traveling
northbound on I-75, take the downtown exit which flows into Erie Street,
turn left on Monroe Street and then Right onto 13th Street. If traveling
southbound on I-75, take the Collingwood Avenue exit. Turn left on Collingwood
Avenue, and then right on Monroe Street. Make a left onto 13th Street. The
parking lot is on the left.Hours

Fees
All services at the Jefferson clinic are offered on a sliding scale (due to a Federal Subsidy), based on your family size and total family income. Each time you arrive for your appointment present a copy of a recent pay stub or other verification of your income. You will be asked some questions about how you support yourself financially. No one is turned away based on inability to pay.
PPNWO accepts Medicaid and other insurances. Please be sure to bring your insurance card (and copay, if applicable) to each appointment.

When traveling
north or southbound on I-475, exit at Airport Highway. Travel east on
Airport Highway, then right on Reynolds Road. Travel south on Reynolds
Road, then left on Glendale Avenue. Take Glendale Avenue until it crosses
Byrne Road, then take a right at the two three story medical office buildings.
Our office is on the second floor in the building on the right. Parking
is available at the front, side, or rear of the building.

Fees
All services at our Glendale location are provided at a fee. Payment is due at the time of service. Medicaid, Buckeye Community Health Plan, and Paramount Advantage are accepted. Please call our office if you have another type of insurance.
